Job Description

The LPN provides direct care for a specific patient population. Administers medications and treatments; provides patient & family teaching, and discharge instructions.

The LPN receives patients in MAP2 clinic in order to insure the effective delivery of service.

This position is scheduled as follows; Monday, Wednesday, Friday 9:00am – 5:30pm;  Tuesday & Thursday 930am – 6:00pm.

Will work at multiple sites when needed (Derm Center, Scarsdale, Bronx East).

RESPONSIBILITIES:

• Provide patient/family teaching and discharge instructions to meet learning/discharge needs as identified by RN.
• Able to set priorities, problem solve and use proper judgment in difficult situations and be flexible in non-emergent situations.
• Provides and maintains safe environment for patients/families through preventive and interventional measures.
• Documents direct patient care as per MMC policies and procedures.
• Participates, practices and promotes continuous quality improvement.
• Communicates effectively with patients/families and members of the health care team. 
• Responsible and accountable for following MMC guidelines.
• Sterilize instruments and equipment using the autoclave. 
• Order and maintain medications and supplies to par level for site/s. Ensure exam rooms are appropriately cleaned and stocked each session, and between patients.
• Collect and properly label specimens as per MMC policy. Maintain specimen(blood,biopsy) logbooks and report abnormal results to MD. 
• Greet and direct patients to exam room.

REQUIREMENTS:

• High School Diploma or equivalent
• NYS LPN license and current registration
• Basic Life Support certification
• Effective English communication skills; written, oral and interpersonal.
• Ability to use basic data entry and retrieval system.
• Additional education required by specialty areas/patient care locations. 
• Excellent interpersonal and telephone skills.
• Automated clinical information system, personal computer, telefax, photocopier, telephone
• Centrifuge, EKG Machine, Sterilization process and/or Autoclave Machine where applicable.
• Misc. Dermatology Surgical Equipment where applies.
• Allergy Equipments where applicable
• Monday, Wednesday, Friday 9:00am – 5:30pm;  Tuesday & Thursday 930am – 6:00pm
