What You'll Be Doing:
Conduct Comprehensive Assessments: Perform annual wellness exams and assessments in patients' homes and virtually.
Educate and Counsel: Provide education and counseling to patients and families about available resources and programs.
Document and Coordinate Care: Maintain accurate patient records and coordinate care with primary care providers and specialists.
Deliver Evidence-Based Care: Provide timely, evidence-based care to reduce hospitalizations and improve patient outcomes.
Prescribe and Collaborate: Prescribe medications, order tests, and collaborate with physicians.
Close Gaps in Care: Educate patients about preventive monitoring and schedule diagnostic testing.
Utilize Technology: Assist patients with accessing educational videos and utilize computer-based data management systems.
Participate in Team Meetings: Collaborate with the integrated care team to ensure coordinated care.
Develop and Implement Treatment Plans: Create and implement personalized treatment plans.
Follow-up and Evaluate: Monitor and evaluate patients' health status.
What You'll Bring to the Table:
Licensure: Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
Certification: Board certification (NP: ANCC/AANP; PA: NCCPA).
DEA Certificate: Current and unrestricted DEA certificate.
Autonomy: Ability to work independently and practice autonomously.
Transportation: Access to reliable transportation, a valid driver's license, and car insurance.
Technical Skills: Proficiency with medical instruments, equipment, and computer-based systems.
Communication Skills: Excellent verbal and written communication skills.
Regulatory Knowledge: Sound understanding of HIPAA and OSHA regulations.
Experience: 2+ years of direct patient care experience.
Preferred Experience: Managed Care/IPA/Health Plan experience and experience conducting annual wellness visits.
Language Skills: Bilingual (English/Spanish) highly preferred.
