Job Description

We are seeking an experienced and dedicated MDS Coordinator (RN or LPN) to join our team. In this full-time role, you'll play a key part in ensuring the accurate and timely completion of MDS assessments, while adhering to all regulatory guidelines and contributing to the success of our care team. Your expertise in clinical assessment and regulatory compliance will be integral in supporting our mission to provide high-quality care.

Key Responsibilities:

Perform and manage MDS assessments in a timely and accurate manner.
Ensure compliance with regulatory requirements and deadlines.
Oversee the data entry and transmission of MDS information.
Troubleshoot and resolve data-related issues.
Prepare and present detailed reports to the Director of Nursing.
Offer feedback and address operational challenges to improve quality.
Participate in facility surveys and audits to maintain high standards.
Assist with audit responses and ensure regulatory compliance is upheld.
Stay informed of the latest Medicare and Medicaid regulations and policies.
Actively engage in quality improvement initiatives related to MDS.
Qualifications:

Graduate of an accredited School of Nursing (RN, BSN, or LPN).
Current and active RN license required.
At least 3 years of clinical experience in long-term care.
Previous experience with MDS/RAI processes is essential.
Strong clinical assessment and critical thinking skills.
In-depth knowledge of Medicare and Medicaid regulations.
Primary series of the COVID-19 vaccine required (specific to location).
